Dark modules on a light background usually provide the strongest contrast. Very similar colours, heavy resizing or insufficient clear space around the code can make scanning less reliable. Higher error correction can tolerate more damage but produces a denser pattern.
The QR image directly stores the text you enter. If it contains a fixed website address, changing that destination later requires a new code unless the address itself redirects somewhere else. Anyone who scans a code can see or open its content, so do not encode passwords or private information.
